Clinical Supports under the Support at Home program and Commonwealth Home Support Program (CHSP) gives you access to qualified health professionals who can help you manage your health, stay mobile, and continue living safely in the place you know best—your home.
These supports may include nursing care, allied health therapies, medication support, wound care, nutrition guidance, and care management.
Comprehensive Clinical Care.
Both the Support at Home and CHSP programs offer comprehensive clinical care (nursing, allied health) and personal support for more complex needs. At Cura, our clinical team walks alongside you with compassion and respect. We take the time to understand your goals, listen to what matters most, and provide the gentle, professional support that helps make everyday life a little easier.
What Clinical Supports Includes.
Clinical Supports brings together three key areas of care to help you stay well and independent at home:
- Nursing care
- Allied health and therapeutic services
- Nutrition and care management
If you receive Clinical Supports through My Aged Care, the amount you contribute will depend on the program you’re approved for. Under CHSP, you may be asked to pay a small co‑payment toward your services. However, if your Clinical Supports are funded through the Support at Home program, these services are fully covered, and you won’t be required to make a contribution.

How our team can support you at home:
Care Management – Home Support Care Management.
Guidance and coordination from your dedicated Care Partner.
Care management is a Clinical Supports included as part of your ongoing support through the Support at Home program. It ensures your care is well‑planned, coordinated, and aligned with your goals, preferences, and cultural needs.
Your Care Partner.
Your Care Partner is your main point of contact. They help you understand your services, organise your supports, and make sure everything runs smoothly. They check in regularly, adjust your care as your needs change, and advocate for you when you need extra help or information.
What Your Cura Care Partner Supports You With:
- Creating and updating your Care Plan
- Coordinating and scheduling services
- Monitoring your wellbeing and adjusting supports
- Communicating with health professionals
- Helping you understand your funding and options
- Supporting you through changes, including hospital discharge or end‑of‑life care
Included
- In‑home support care management
- Planning, coordinating, reviewing, and monitoring your services
Excluded
- Administrative costs such as billing, claiming, and account management
Care Management helps ensure your care stays connected, consistent, and centred around what matters most to you.
By contrast, the CHSP provides entry‑level, low‑intensity support and generally does not include ongoing, formal care management as part of its service offering.
Restorative Care Management.
Short‑term, goal‑focused support to help you regain independence.
Restorative Care Management is a key component of the Restorative Care Pathway under the Support at Home program. It brings together allied health and nursing professionals to help you rebuild strength, mobility, and confidence after a setback.
Your Care Partner coordinates your program, monitors your progress, and ensures your supports are tailored to your goals so you can get back on your feet and continue living independently at home.
The Support at Home program also offers three dedicated short‑term care pathways, each designed for different stages of need. Restorative Care Management is not a standard feature of CHSP.

Who is Clinical Supports for?
Clinical Supports is designed for seniors who would benefit from professional health services to stay independent at home. You may find Clinical Supports helpful if you:
- need help managing a chronic or complex health condition
- would like to improve your nutrition or overall wellbeing
- require physiotherapy or exercise support to maintain mobility
- need nursing assistance with medication or wound care
- want guidance from allied health professionals to stay safe and active
These services are part of the Australian Government’s Support at Home program and are focused on keeping you well in your own home for as long as possible.
Who Is Eligible for Clinical Supports?
You may be eligible if you:
- are aged 65+ or older (50 years or older for Aboriginal and Torres Strait Islander people)
- Special circumstances: People who are homeless or at risk of homelessness may also be eligible from 50 and
- have noticed a change in what you can do or remember
- have been diagnosed with a medical condition or reduced mobility
- need help with daily tasks or managing your health
- have been assessed through My Aged Care or wish to access services privately
How do I Access Government Funding for Clinical Supports?
To access government‑funded clinical supports, you’ll need to register with My Aged Care and arrange an assessment.
