Short-Term Care Options.

Short‑term care can make a meaningful difference during times of change. Whether you’re recovering from an illness or hospital stay, adjusting to new health needs, or seeking comfort and support toward the end of life, our short‑term pathways are designed to help you feel safe, supported, and cared for.

These programs offer focused, time‑limited support that helps you regain strength, maintain independence, or experience comfort and dignity at home when it matters most.

Restorative Care.

Short-term support focused on helping you regain strength, mobility, and independence through allied health services. Support is available for up to 12 weeks, with a possible 4week extension depending on assessment.

End-of-Life Care.

Compassionate, in-home support for those navigating the final stages of life. Support is available for people with a prognosis of three months or less, helping you remain at home for as long as possible.

Assistive Technology & Home Modifications (AT-HM) Scheme.

Eligible participants can access assistive technology and home modifications without needing to save from their individual budgets. This service assists in adapting your home for improved safety and comfort.
