Care Management is a core part of the Australian Government’s Support at Home program [link Support at home page]. It ensures your services are well‑planned, coordinated and continually aligned with your goals, preferences, cultural practices and changing needs.
At Cura, your Care Partner becomes your trusted point of contact — someone who understands your situation, helps you navigate the aged care system, and supports you to stay safe, independent and connected at home.
Care Management Services Can Include.
Your Care Partner provides ongoing support to help you manage your care with confidence.
This includes:
Care Planning
Developing, reviewing and adjusting your Care Plan and budget
Service Coordination
Organising your roster of support workers, clinicians and allied health professionals
Monitoring & Evaluation
Checking in regularly to ensure your services remain safe and appropriate
Risk Management
Identifying changes in your health or home environment and responding early
Support & Education
Helping you understand your services, funding and options
Advocacy
Speaking on your behalf and ensuring your needs are heard
Communication With Your Care Team

Care Management Services Exclude.
Care Management does not include:
- Administrative tasks such as billing, claiming or account management
- GP visits, specialist appointments or hospital treatments
- Direct personal care or clinical services (these are delivered by your support workers or clinicians)

This may involve:
- Community programs such as social groups, volunteering, dementia‑friendly activities or cultural organisations
- Care circles that bring together your support workers, clinicians, family and community members
- Compassionate, ongoing support that adapts as your needs, preferences or circumstances change
- By nurturing community connections, we help you maintain independence, reduce isolation and feel supported in every aspect of your life.
Culturally Sensitive Care.
Your cultural identity is an important part of who you are. Cura ensures your care is delivered in a way that feels familiar, respectful and culturally safe.
Your Care Partner will:
- Ask about your cultural preferences and incorporate them into your Care Plan
- Support gender‑specific or culturally appropriate care arrangements
- Use your preferred language where possible, with interpreter support if needed
- Respect cultural beliefs that influence your care decisions
- Work closely with your family or community supports
- We honour your culture, traditions and values in every step of your care journey.
Your Care Partner communicates your cultural needs to your Home Care Worker team, ensuring your care feels familiar, respectful, and safe.
How Can I Access This Service?
Care Management is included for all older people receiving ongoing services through the Support at Home program.
To access it:
Register with My Aged Care.
An Aged Care Assessor will determine your eligibility.
Once approved, you will choose an aged care provider — such as Cura — who will assign you a dedicated Care Partner.

FAQ's.
What is the role of a Cura Care Partner?
Your Care Partner coordinates your services, reviews your Care Plan, monitors your wellbeing and helps you understand your funding and options.
Is Care Management included in my funding?
Yes. Care Management is included as part of your ongoing government subsidised Support at Home services.
How often will my Care Partner check in?
Check‑ins are based on your needs and preferences. This may include phone calls, home visits or reviews after changes in your needs or health.
Can my Care Partner help me access other supports?
Yes. They can connect you with clinical services, allied health, community programs, respite or other aged care supports.
What happens if my needs change?
Your Care Partner will update your Care Plan and adjust your services. They can also arrange reassessments if you require additional support.
Can I receive Care Management if I am funded through the Commonwealth Home Support Programme (CHSP)?
Care Management is not included as a standalone service under CHSP. CHSP provides entry‑level, occasional support and does not offer ongoing care coordination. If your needs increase and you require regular planning, monitoring or coordination, you will need to be referred for a reassessment for the Support at Home program, where Care Management is included as part of your ongoing services.
