When your health changes unexpectedly — after a hospital stay, a fall, or a period of feeling unwell — it can be hard to find your footing again. Restorative Care Management is designed to support you through this time with a structured, short‑term program that focuses on regaining function and returning to daily life safely.

Delivered through the government’s Support at Home [link to Support at home page] program, this pathway provides dedicated funding and a coordinated approach to rehabilitation. Your Care Partner works closely with you, your family, and allied health professionals to create a plan that reflects your goals, your culture, and the way you prefer to live. Together, we focus on early intervention, rebuilding confidence, and preventing further decline so you can continue living independently at home.

Access to the Restorative Care Pathway is determined through a My Aged Care assessment and outlined in your support plan. Once approved, you’ll receive time‑limited, goal‑focused support designed to help you recover, strengthen your abilities, and feel more in control of your everyday life.

What Support Can Restorative Care Management Include.

Restorative Care Management may include:

Goal Based Care Planning

Identifying what matters most to you, such as improving balance, returning to daily tasks, or rebuilding confidence

Coordination of Allied Health Services

Organising physiotherapy, occupational therapy, exercise physiology, nursing, and other clinical supports

Monitoring & Reviewing Progress

Ensuring your services are working well together and adjusting your plan as your needs change

Assistive Technology & Home Adjustments

Arranging equipment or minor modifications to support safety and independence

Education & Advocacy

Helping you understand your program, your goals, and your transition plan when the pathway ends

Services Exclude from Restorative Care Management.

  • Administrative costs such as billing, account management, or claiming
  • GP or specialist appointments (these are accessed through Medicare)
  • Long‑term or ongoing care management outside the restorative period
  • Services for people other than the person receiving restorative care

How This Service Can Help.

Restorative Care Management is designed to support your recovery by:

Most programs run for 12–16 weeks, with dedicated funding of $6,000, which may increase to $12,000 if clinically required. This funding is separate from your regular Support at Home budget.

Cura recognises that recovery is shaped by culture, family roles, and personal beliefs. Your restorative care is planned with these in mind. We take the time to understand your routines, preferences, and cultural needs, and wherever possible, match you with professionals who understand your background. This ensures your care feels respectful, familiar, and supportive throughout your recovery journey.

How Can I Access This Service?

Restorative Care Management is available through the Support at Home program. To access it, you must:

65 years or older.

50 years or older – if they are an Aboriginal or Torres Strait Islander person, or homeless or at risk of becoming homeless. If they are under 65, people in these two categories must also be told about other services that may be available to them before having an aged care assessment.

Have experienced a recent decline in function due to illness, injury, or hospitalisation.

Be registered with My Aged Care.

Get assessed by an My Aged Care Assessor and receive approval for this service.

You may be able to access the Restorative Care Pathway up to twice in a 12‑month period, with a minimum 90‑day gap between episodes.
