The new Support at Home program, along with the Commonwealth Home Support Programme (CHSP), is a key pathway to subsidised care. Your first step is to contact My Aged Care, either online or by phone, to arrange an assessment. Once assessed, you will be assigned a funding level based on your needs. Cura Aged Care then works closely with you to understand your specific requirements and develop a tailored care plan that maximises your funding. We specialise in helping clients understand their entitlements, apply for packages, and manage their funds effectively, ensuring seamless access to the essential Home Care Services you need without unnecessary stress or confusion, positioning us as your trusted expert for funding navigation.
Yes. If you’re currently receiving services through Support at Home or another government-funded aged care program, you can choose to switch your provider to Cura Aged Care.
The process is generally straightforward, although the timeframe will depend on the terms and notice period outlined in your current provider’s service agreement. The first step is to notify your current provider that you wish to end your services and agree on a transition date.
Once your services have ended, you’ll need to contact My Aged Care to have your package referral code reactivated and made available to a new provider. While Cura Aged Care cannot complete this step on your behalf, our team can provide guidance and support throughout the process.
Once your referral code is available, Cura Aged Care can accept the referral and work with you to create a personalised support plan tailored to your needs, goals and preferences. We’ll help make the transition as smooth as possible, so you can continue receiving the support you need with minimal disruption.
